Affidavit for express entry

As part of express entry application, you have to submit various documents like educational (degree and marksheets of undergraduate and graduate degrees), proof of funds and employment letters etc. Note that the name on these documents. The name on different documents might be different than what is on your passport.

  1. You officially changed your name.
  2. Different organizations use different conventions eg:  <First> <Father's name> <Mother's name> <Surname>
  3. Plain error from the organization issuing the certificate.
While you cannot change your originals, what you can do is create an affidavit for cases where there is deviation from the passport name.


Why this is important?
The express entry process is practically online. If the documents have different names, the officials would want to know that all the documents belong to you. Without the affidavit, the officials might want to do additional checks to rule out fraud. You can avoid hassles or requests of additional detail with the affidavit.

What is an affidavit?
A written statement of facts, sworn to and signed by a deponent before a notary public or some other authority having the power to witness an oath.

What you have to do in this case?
Declare the different names in different documents. Sign in front of a notary. The notary will sign, put their seal and date it. Your affidavit is ready.

While submitting your express entry documents (i.e. post ITA), it will ask you if you have used different names. You should answer YES and then upload this affidavit.

Where to find a notary?
Some folks at work might be certified notaries. Ask around at work and you could get it done for free.
In US, stores like UPS and Fedex notarize documents at a nominal charge.
