Affidavit is a legal document by which you are making some information about yourself legal. In the context of express entry application, if you have had different name formats on different documents due to any reason then you can create an affidavit that declares you as the owner of those documents. On popular request, I have shared an example below of an affidavit template. In the example below, I have
used John Doe as an example of a person’s name. Shawn and Marie are first names
of parents in the example. Look thru your documents and where ever there is a
deviation from the name in the passport list it in the affidavit. In the
example below, there are different variations. Depending on your case, put in
all variations (see examples below). If you want to state any other fact
legally you can do so. Remember that affidavit is just a statement of facts and
you sign it only in front of a notary. It becomes legal ONLY when you sign in front of a notary and the notary also signs, seals and dates the document in front of you.
